STEP 01
Let caregivers prepare in their own words.
Send age-appropriate intake and supported developmental questionnaires, including PSC-17 and M-CHAT-R. Caregiver responses are attributed to the person who supplied them.
Explore each step. Your team stays in control.
HOW THE WORK MOVES
Your Care Pack, from intake to follow-up.
Specialty workflows built on one patient record—not just a collection of note templates.
- 01
Let caregivers prepare in their own words.
Send age-appropriate intake and supported developmental questionnaires, including PSC-17 and M-CHAT-R. Caregiver responses are attributed to the person who supplied them.
- 02
See the child’s progress in context.
Age-band well-child and sick-visit templates work with CDC/WHO growth percentiles and immunization tracking. The clinician reviews screening inputs and results in the chart.
- 03
Keep the next visit visible.
Surface missed well visits, immunizations due and screening follow-up as attention items. Guardian consent and messaging rules shape how the clinic reaches the family.
CARE THAT GROWS WITH THE CHILD
One family story, across every well visit.
Growth data, caregiver context and the next recall belong to the same patient story. The team can review what is due and decide the appropriate action.

Caregiver context before arrival
Age-appropriate intake keeps answers attributed to the caregiver who supplied them.
Growth and screening in context
Review growth percentiles, immunization history and supported developmental screening alongside the visit.
The next milestone stays visible
Missed well visits, immunizations due and screening follow-up become attention items shaped by guardian consent and messaging rules.
CLEAR RESPONSIBILITIES
Clinical decisions stay with your clinicians.
Care Packs provide specialty intake, documentation, offered scores, treatment-plan recalls and attention worklists. Scores use published definitions and stated inputs; clinicians choose when to use them. Clinical proposals require review. Specialty interfaces and local protocols are confirmed during setup.

A few useful answers.
Is ambient scribing included in the plan?
Ambient scribing is an optional $69 per provider per month add-on, scoped to the provider across clinics. It is not included in the base plans. Patient consent and clinician review are required.
Does caregiver intake preserve who answered?
Yes. The guardian model supports consent and messaging, and caregiver-supplied answers remain attributed to the caregiver. Confirm your practice’s access and confidentiality policies during setup.
Can we bring our existing forms and records?
Existing PDF forms can become conversational intake. C-CDA imports support key clinical records; import completeness depends on the source export and should be validated before everyday use.
Do we have to replace all our systems at once?
No. Discuss the workflows you want to improve first and the tools you plan to retain. We will confirm supported handoffs and the scope of a staged adoption.
